Reward Excellence bonus

Staff positions qualify for the individual quarterly, department and clinic bonuses. Supervisors and above qualify for the department and clinic bonuses. Shown as a separate opportunity, not part of the total.

Bonus maximums are prorated by FTE. 1.00 is a full allocation.

Continuing education and licensure

Covers CEU tuition, travel, license renewals and board certification under the Continuing Education and Licensure Benefit policy.

Employer-paid insurance

Lexington Clinic pays for basic life insurance at two times your salary and for short and long-term disability. The premium cost is estimated until the Benefits Department provides the carrier rates.

What this measures

Annual gross pay plus the value of what Lexington Clinic pays toward your benefits. It is not take-home pay, a complete employer cost statement or an offer of employment. Taxes, overtime, shift differentials and the tobacco and working-spouse premium surcharges are not modeled. Annual values assume 52 weeks of pay and 26 biweekly premium deductions.

    Plan rules that apply

    • Retirement: 5% of eligible pay up to the IRS compensation limit of $360,000 for 2026, made whether or not you contribute. The 3% basic contribution is fully vested. The 2% profit sharing contribution vests over qualifying years of 1,000 hours: under 2 years 0%, 2 years 20%, 3 years 40%, 4 years 60%, 5 years 80%, 6 years 100%. Amounts shown are employer allocations, not vested balances, and use base pay only.
    • HSA: Lexington Clinic matches 50% of your contributions up to $500 for employee-only coverage or $1,000 for all other coverage levels, on the HDHP only.
    • Reward Excellence: 2026 potential of $600 per quarter for the individual bonus, $800 for the department bonus and $600 for the clinic bonus, after three months of employment. Quarterly bonuses pay on the last check of March, June, September and December; annual bonuses pay in December.
    • Basic life and AD&D: two times annual salary up to $1,000,000. Short-term disability: 30% of pay up to $2,500 a week for up to 12 weeks, starting the eighth day. Long-term disability: 60% of base pay up to $8,000 a month after 90 days. The guide lists disability as effective both on the date of hire and on the first of the month after six months of full-time employment, so no start date is asserted here.
    • Continuing education: $400 and $800 categories require six months of service; $1,500 and $3,000 categories are eligible on hire. The $3,000 category covers 2026 and 2027 together. Positions of 20 to 29 hours receive half. Unused amounts do not carry over.
    • Eligibility: full-time benefits begin at 30 hours per week. Medical, dental, vision and life coverage start on your first day.

    Benefits beyond the total

    More ways Lexington Clinic supports you. These benefits are available to you, but their dollar value is not added to the total above.

    1. Health and protection

      Preventive care is covered in full when performed by a Lexington Clinic provider, and Tier 1 access to Lexington Clinic providers means a $10 primary care copay and a $20 specialist copay on the PPO with lab work and radiology covered in full. Dental and vision plans, a healthcare FSA and a dependent care FSA are available, medical, dental and vision premiums come out of your pay before taxes, and tobacco cessation products are covered at no cost on every medical plan.

    2. Time and professional growth

      Paid time off and holidays, plus two to five paid days a year to attend continuing education courses for roles with a CE allowance. Paid leave is already reflected in your pay, so it is shown but not added. Approved online courses can be completed on the clock, and educational reference materials and job-related association dues are reimbursable within your allowance.

    3. Support for everyday life

      The Employee Assistance Program, paid by Lexington Clinic, provides three face-to-face counseling visits and confidential resources for you and your household family members. Through The Hartford you also have travel assistance, identity theft help, counseling through Ability Assist and online will preparation at no cost, whether or not you enroll in the health plans.
